The initial presentation and in hospital treatment
EMS:
You are called to a 7 y/o female, complaining of an unremitting headache. Pt is currently experiencing nausea and vomiting periodically, no blood noted, and is responsive only to verbal stimuli at this time. Sluggish pupillary response at this time. Pt's mother reports that the illness was sudden onset in the last 36 hours and pt was pulled from school; no has been sick in the family or at school that she knows of. Pt was brought to urgent care yesterday and diagnosed with streptococcal pharyngitis and received a penicillin shot
Last set of vitals: HR 136 strong and regular and radial, BP 110/66, RR 30 nonlabored, SpO2 96% on room air, temp 101.6 F.

In Hospital:
Pt spent approx 4 hours in the ER before being admitted to pediatric intensive care unit after having her first tonic-clonic seizure. Pt lost consciousness shortly after admission. On secondary assessment, no rash noted although pt is slightly erythematous overall.
When grilled for a more thorough history: mother states that the pt has not eaten anything that she hasn't eaten and she is not reporting any symptoms. The pt is completely up to date on vaccinations, even received her flu shot last fall. whole family went camping two weeks prior to the onset of the illness. Grandpa also died of a stroke a month ago.
